SCCIPA Medicare Advantage Member Services

Whether you are newly eligible for Medicare or transferring from a Medicare supplemental plan, you may have questions about Medicare Advantage plans and Medicare Part D prescription drug coverage.

We contract with Seven Medicare Advantage HMO Plans:

Compared to Medicare Supplemental plans, these plans can save you several thousand dollars a year with benefits like:

  • Part D Prescription drugs
  • Routine physical exams
  • Vision services
  • Dental
  • Transportation
  • Over-the-Counter benefits

Anyone with Medicare Part A and Part B may apply for these plans. Members continue to pay Medicare premiums and use contracting providers for routine care. Limitations and copayments may apply. Please contact the plans directly for specific information about benefits.

You may also contact HICAP at 408-296-8290 if you need help comparing Medicare health plans. HICAP (Health Insurance Counseling and Advocacy Program) is a statewide, nonprofit program that provides FREE counseling to California seniors who need help evaluating alternative Medicare plans. HICAP is unbiased and will assist with comparison of plans that best fit your individual needs. Volunteer counselors are conveniently located throughout Santa Clara County.

Annual Wellness Visit

Our goal at Santa Clara County IPA (SCCIPA) is to provide the highest level of care to meet your unique needs. As a SCCIPA member, you are eligible for an Annual Wellness Visit, at no cost to you. This comprehensive yearly visit is important to your overall health, managing existing health conditions, and may help prevent illness to keep you leading an active healthier life.

We encourage you to take advantage of this Medicare benefit and schedule an office visit with your PCP. This is the perfect opportunity to:

  • Discuss your lifestyle, family history and medical history.
  • Have the doctor review all your current conditions and medications.
  • Receive personalized preventative care. Early screening for diseases makes them easier to treat.
  • Discuss any concerns you may have.

Please take the time to fill out the Patient Health Risk Assessment + History survey and bring it to your office visit, once it is scheduled. This will help your doctor have a better understanding of your health needs. If you forget to bring the survey, your doctor’s office can provide you with a new one.
